Compliance audits do not fail because of sophisticated attacks or exotic vulnerabilities. They fail because of operational gaps that most security teams know about but never close. Auditors are not looking for perfection; they are looking for evidence that your controls exist, operate consistently, and produce the outcomes your policies promise. The gap between what your documentation says and what your environment does is where failures live.
After supporting dozens of SOC 2 and ISO 27001 audits across mid-market companies, the same ten issues appear with striking regularity. These are not edge cases. They are systemic failures that stem from a lack of process discipline, not a lack of security knowledge. Understanding what auditors actually test, and how they test it, is the first step toward a clean report.
Access Reviews, Training, and Incident Response Gaps
The single most common audit failure is incomplete or missing user access reviews. Every major framework requires periodic validation that user access rights are appropriate for current job responsibilities. Auditors pull a sample of employees and ask for evidence that someone reviewed their access within the defined period, typically quarterly or semi-annually. If you cannot produce timestamped records showing who reviewed access, what was reviewed, and what action was taken, you have a finding. The fix is not a better tool; it is a recurring calendar event with an accountable owner and a documented output.
Security awareness training failures rank second. The control is straightforward: all employees must complete security awareness training within a defined period of hire and annually thereafter. Auditors check completion records against the employee roster. If three out of fifty employees never completed training, that is a finding regardless of the reason. New hires who joined mid-quarter, contractors on short engagements, and executives who skipped the email all create gaps. Vendor risk assessments are required whenever you share data with or depend on a third-party service. Auditors ask for a vendor inventory, risk tiering criteria, and evidence of periodic assessment. Most organizations can produce an initial vendor assessment but fail to reassess vendors on the defined cycle, typically annually. When the auditor asks for the most recent assessment of your cloud hosting provider or payroll processor and the only document is from the original procurement two years ago, that is a finding. Maintaining a vendor register with assessment dates and next-review triggers eliminates this issue.
A stale risk register is a subtler but equally common failure. Frameworks require organizations to identify, assess, and treat information security risks. Auditors expect the risk register to reflect current threats, not a snapshot from the initial program build. If your risk register lists the same twelve risks with the same ratings and the same treatment plans from eighteen months ago, the auditor will question whether risk management is an active process or a checkbox exercise. Quarterly risk register reviews with documented updates, even if the conclusion is no change, demonstrate that the process is alive.
Evidence Gaps, Policy Mismatches, Encryption, and Business Continuity
Evidence gaps are the operational cousin of policy mismatches. Your policy says you perform vulnerability scans monthly. The auditor asks for twelve months of scan reports. You produce nine. That three-month gap is a finding, and the explanation does not matter. Evidence collection must be treated as a continuous process, not a pre-audit scramble. The organizations that pass audits cleanly are the ones that collect evidence in real time using a GRC platform, a shared drive with a defined folder structure, or even a spreadsheet with links. The medium does not matter; the consistency does.
Policy and practice mismatches are the most avoidable failure on this list. If your password policy requires fourteen-character passwords with complexity requirements but your identity provider enforces eight characters, the auditor has a finding before they leave the documentation review. The solution is not writing aspirational policies; it is writing policies that reflect what you actually enforce and then configuring your systems to match. Auditors want evidence of a test, typically an annual failover exercise, tabletop walkthrough, or recovery drill, with documented results and remediation of any issues identified. If your DR plan references a secondary data center that was decommissioned six months ago, or if your RTO and RPO targets have never been validated through an actual recovery exercise, the auditor will flag it. A single documented DR test per year, even a partial one, demonstrates operational commitment to resilience.
